This notice explains what Preferred Health Agency LLC (trading as Preferred Health Insurance Agency LLC, "we") does with personal information collected through this website. Our office is 1560 Latham Road, Suite 6, West Palm Beach, FL 33409 and you can reach us at info@preferredhealthandlife.com.

The short version. We collect what you type into the quote form plus some technical data about your visit. We use it to have a licensed agent contact you about coverage and to measure our advertising. We give it to the service providers who make that possible, and to the licensed agents and carriers needed to quote and place the coverage you asked for. You can ask us to stop, and you can ask us to delete it.

1. What we collect

Information you give us

Contact details — first and last name, phone number, email address.
Location — ZIP code, and the state it falls in.
Coverage details — who the coverage is for, the kind of plan you are looking for, what matters most to you, and optionally date of birth.
Your consent record — the exact wording of the consent you agreed to, its version, and the date and time you agreed. We keep this so that if there is ever a dispute about whether you agreed to be called, the answer is a matter of record rather than opinion.

Information collected automatically

IP address, browser and device type, pages viewed and time on page.
Advertising identifiers — including the Google click identifier (gclid) and campaign parameters — which tell us which advertisement brought you here.
If you call a number shown on this site, the tracking number you dialled, the time and duration of the call, and your caller ID.

2. Sensitive information

Your interest in health coverage, and any health details you volunteer, are treated as sensitive personal information under several state privacy laws. We use it only to provide the insurance services you asked us for. We do not use or disclose it to infer characteristics about you, and we do not use it for advertising.

Please do not send us medical records or diagnoses through this website. If a carrier needs medical information for an application, they will collect it directly through their own process.

3. Why we use it

To have a licensed agent contact you about the coverage you asked about.
To prepare quotes and, if you choose to apply, to submit an application to a carrier.
To measure which advertising works, and to stop showing ads to people who have already contacted us.
To detect fraudulent or duplicate submissions.
To meet our obligations under insurance, telemarketing and other applicable law, including keeping records we are required to keep.

4. Who we disclose it to

We disclose personal information to:

Service providers acting on our instructions — our contact-centre and dialling platform, our call-tracking provider, and our website hosting and email providers. They may use it only to perform services for us.
Licensed insurance agents and insurance carriers, where that is necessary to quote or place the coverage you requested.
Advertising and analytics providers, described in section 5.
Regulators, and others where the law requires it — including a state insurance department reviewing a complaint.

We do not publish or auction your contact details to unrelated advertisers.

5. Advertising, and your right to opt out of "sharing"

This site uses Google advertising and analytics tags and a call-tracking script. Under California law and several other state laws, disclosing information to advertising partners for cross-context behavioural advertising counts as "sharing" (and in some definitions, a "sale") even when no money changes hands. Because of that, you have the right to opt out — and we honour it whether or not the law in your state grants it.

We also honour the Global Privacy Control browser signal. If your browser or extension sends it, we treat it as an opt-out request for that browser without you needing to contact us.

Opting out of advertising does not stop a licensed agent from contacting you about a quote you requested. To stop calls and texts, see Do Not Call.

6. Your rights

Depending on where you live, you may have the right to:

Know and access the personal information we hold about you, and how we got it.
Correct information that is inaccurate.
Delete your information, subject to records we are legally required to keep.
Receive a portable copy of information you provided to us.
Opt out of targeted advertising, of sale or sharing, and of profiling used to make significant decisions about you.
Not be discriminated against for exercising any of these rights. We do not offer financial incentives in exchange for your data.

How to exercise them

Email info@preferredhealthandlife.com or write to 1560 Latham Road, Suite 6, West Palm Beach, FL 33409. Tell us what you want and give us enough detail to find your record — normally the phone number or email address you submitted. We will verify who you are before we act, and we may decline a request we cannot verify. An authorised agent may act for you with written permission.

We respond within 45 days, and will tell you if we need a further 45 days.

If we say no — your right to appeal

If we refuse a request, you may appeal by replying to our decision with the word "appeal". A different reviewer will look at it and we will respond in writing within 45 days, explaining our reasoning. If we still refuse, we will tell you how to complain to your state attorney general.

7. Insurance privacy rules also apply

As a licensed insurance agency we are also subject to state insurance privacy regulations derived from the NAIC model rule and the federal Gramm-Leach-Bliley Act. Those rules govern non-public personal financial and health information collected in connection with an insurance transaction, and they give you the right to limit certain disclosures to non-affiliated third parties. Where those rules and the state privacy laws above both apply, we give you the benefit of whichever is stronger.

8. Cookies and tracking

We use cookies and similar technologies to keep the form working, to remember which advertisement referred you, and to measure performance. You can block or delete cookies in your browser settings; the quote form will still work, but our advertising measurement will be incomplete. See section 5 for the advertising opt-out.

9. How long we keep it

We keep quote enquiries and the associated consent records for as long as we may need them to service the relationship and to meet the record-keeping periods that insurance and telemarketing rules impose on us, and then we delete or de-identify them. Consent records are retained for as long as we could be asked to prove consent.

10. Security

We use access controls, encryption in transit, and vendor agreements to protect personal information. No method of transmission over the internet is completely secure, and we cannot guarantee absolute security.

11. Children

This site is not directed to children and we do not knowingly collect personal information from anyone under 18. A parent or guardian may include a minor child on a family quote request.

12. Changes

If we change this notice we will update the date at the top. Material changes will be highlighted on this page.
